Job Summary:
The Patient Care Coordinator’s responsibility is attaining authorization for therapy services provided by Clinic facilities along with the intake of referrals, pre-certification or verification of benefits and physician orders. The coordinator will provide information regarding services to providers and parents. Serve as a liaison between clinical staff, patients, and physicians. Receive telephone intake for therapy services. Maintain clear and concise scheduling for clinic therapy staff’s patients. Complete proper documentation in a timely manner.
Responsibilities:
- Obtaining authorizations for Clinic therapy services.
- Scheduling/Coordinating Clinic therapy services.
- Effectively tracking authorizations to ensure services are not furnished without authorization.
- Effectively communicating information between multiple agencies, insurance companies, patient family, and clinicians
- Documenting clear follow up with insurance company, physicians, patient family, and Clinic therapists.
- Data entering patient information.
- Maintaining Excel spreadsheet(s)
- Provide accurate intake information on all referrals.
- Obtain pre-authorization if required by payor.
- Contact physicians for orders on all new referrals; contact physicians for approval and orders for changes in existing patient Plan of Treatment
- Effectively track & document referrals to ensure services are scheduled in a timely manner.
- Relay messages from clinic staff to office personnel as needed.
- Provide general information regarding clinic services to the community and physicians.
- Notify Director regarding admissions and re-admissions.
- Provide a professional and consistent quality of performance in all assigned tasks.
- Maintain orders for clinical staff members following Clinic policies and procedures.
- Prepare and provides daily reports to track all referrals, pending admits, and evals.
- Additional responsibilities as requested.
Requirements:

- Ability to write routine reports and correspondence.
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
- Must be able to read 12 point or larger type.
- Basic computer skills to include Microsoft Office, Excel, Outlook.

Requirements:
- Ability to learn to use our agency software for electronic medical records.
- Experience using and working with medical terminology
- Bilingual Preferred
