Circle of Care Pediatric Home Healthcare Company is looking for individuals to fill immediate positions for a Physical Therapist for patients in the home, daycare or community setting as prescribed by the attending physician.
Circle of Care Pediatric is a home health services agency that focuses on providing in home and community based therapies for children up to 20 years old. We are committed to providing a full Circle of Care to your child.
The physical therapist is a qualified professional person who directs, supervises, evaluates, and provides therapy services to patients in the home as prescribed by the attending physician.
- Graduate of a physical therapy school approved by The American Physical Therapy Association
–or– The Council on Medical Education of Hospitals for the American Medical Association Licensed or registered by the state of Texas
- One (1) year institutional experience, preferred
Prepares an assessment of the patient’s condition and periodic evaluations and summaries, develops a plan of care and problem list, and writes clinical/progress notes on the date of the visit and incorporates the same in the patient’s record no less than weekly.
Participates in the development and periodic revision of the physician’s plan of treatment
Assists the physician in evaluating patient’s by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests.
Records and reports to the physician the patient’s reaction to treatment and/or any changes in the patient’s condition.
Instructs patients and their families in the use of prosthetic, orthotic, and assistive devices (canes, wheelchairs, etc.).
Instructs the family in the patients total physical therapy program.
Attends case conferences and in-service programs as indicated.
Supervises the physical therapy assistant, and evaluates the effectiveness of the services furnished by reviewing the assistant’s notes.
Participates in staff and Agency development activities.
Confirms, on a weekly basis, the scheduling of visits with the Patient Care Coordinator.
Notifies Agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, which will affect agreed services with the Agency.
Participates with staff, patient, and physician in discharge planning activities and completes the physical therapy discharge summary within the designated framework of the Agency policies.
